The Centers for Medicare & Medicaid Services (CMS) approves the survey vendors listed below to administer the ACO Realizing Equity, Access and Community Health Consumer Assessment of Healthcare Providers and Systems (ACO REACH CAHPS) Survey. Survey vendors with an asterisk (*) next to their name conduct both the Standard/New Entrant and High Needs surveys.

Please note this is the final list of approved vendors for Performance Year 2024 (PY 2024). All vendors listed are fully approved and available for authorization.

The vendor authorization window for the PY 2024 ACO REACH CAHPS Survey is June 7-August 2, 2024. All ACOs are required to authorize a vendor.
