Update to ACO REACH CAHPS High Needs Telephone Script

This email serves to inform survey vendors that we have posted an updated version of the ACO REACH CAHPS High Needs Telephone Script (English) to correct an error. The updated version of the High Needs Telephone Script (English) changes the wording on Q54Intro to match the wording on Q48Intro in the Standards/New Entrants Telephone Script, “The next questions ask if, because of a health or physical problem, (you are/[FNAME] [LNAME] is/your [PROXY_RELATION]) is) unable to do, or have any difficulty doing daily activities.”

This has been corrected in the High Needs Telephone Script (English). Please discard the previously posted version and use the current version. We thank our vendors for helping us spot this error.
