2025 ACO REACH CAHPS Survey Vendor Application Period Closes Soon

The Performance Year (PY) 2025 survey vendor application period for ACO REACH CAHPS will close on January 21, 2025.

To apply to become an ACO REACH CAHPS Survey vendor, organizations must:

Meet all the Minimum Survey Vendor Business Requirements at the time of application submission, including being approved for mixed mode (mail with telephone follow-up) by another CMS CAHPS survey.
- Vendors who are currently operating remotely for any portion of their mail and/or telephone operations and wish to continue remote operations for ACO REACH CAHPS must submit an Exceptions Request Form (ERF) to CMMI for review for approval.
- The online ERF form is available for organizations that have registered for vendor credentials. Submit the ERF after submitting the vendor application. The ERF is located under the "Vendors" tab once vendor applicants have logged into the ACO REACH CAHPS website.
Previously approved ACO REACH CAHPS survey vendors need only update their approved remote operations ERF if they have made changes to their remote operating protocols since it was last approved.
Complete and submit the application form by January 21, 2025.

REACH ACOs will receive a list of conditionally approved ACO REACH CAHPS survey vendors in April 2025 and fully approved ACO REACH CAHPS survey vendors in June 2025.

The ACO REACH CAHPS Final Data Submission for Performance Year 2024 deadline is TODAY

The ACO REACH CAHPS final data submission for performance year (PY) 2024 deadline is TODAY, Monday January 13, 2025.

We thank the ACO REACH CAHPS survey vendors who have already submitted PY 2024 final data files for their client REACH ACOs. If you have not yet submitted PY 2024 final data files for your client REACH ACOs, please submit your file(s) by 11:59 PM ET today, January 13, 2025.

Important Note: Since CMS will not accept any final files after today, January 13, 2025, please make sure you check your data submission reports today to verify that all your submitted files were accepted. If needed, correct, and re-submit all files today.

For information about data file preparation and submission processes, please review Chapter 7 of the ACO REACH CAHPS Quality Assurance Guidelines (QAG) Manual.

The ACO REACH CAHPS Survey Coordination Team will review all submitted files immediately after the deadline. We will run a series of edits on survey data to check for issues such as inconsistencies and patterns. Although CMS does not permit data submissions after the January 13, 2025 deadline, we will contact vendors with identified issues.

Survey Vendor Application is open for ACO REACH CAHPS Survey for 2025
Application Information & Minimum Business RequirementsPosted December 16, 2024

The application to become a CAHPS survey vendor for the Performance Year (PY) 2025 ACO REACH CAHPS is now open and will remain open through January 21, 2025. Vendors who meet all of the Minimum Survey Vendor Business Requirements at the time of application submission, including being approved for mixed mode (mail with telephone follow-up) by another CMS CAHPS survey, are invited to apply. Applications must be submitted by January 21, 2025.

New vendor applicants must follow the directions below to submit their application.

First, register for login credentials. Vendor Registration is located under the “Vendors” tab on the website to gain access to the private side of the website (https://acoreachcahps.org/).
Then, complete and submit an online Vendor Application. The Vendor Application is located on the Vendor Dashboard after logging in.

Vendors with a previously approved application do not need to create new login credentials, nor start a new application. To complete this task:

First, login with your current credentials.
Then, click on “Edit/View Vendor Application” on your Vendor Dashboard. You will be directed to your current vendor application, which you can review, and update based on any changes for PY 2025.
Even if you have no changes to your vendor application, please review all parts, update part 7 (date and sections updated), and submit the application.

For more information about the ACO REACH CAHPS survey vendor responsibilities, please review the FAQs for Vendors located in the Quick Links box of the website and the PY 2025 schedule located on the home page. REACH ACOs will receive a list of conditionally approved ACO REACH CAHPS survey vendors in April 2025 and fully approved survey vendors in June 2025.

The ACO REACH CAHPS Final Data Submission Window for Performance Year 2024 is now open through January 13, 2025

The final data submission window for ACO REACH CAHPS Survey Performance Year (PY) 2024 is now open and will remain open through Monday January 13, 2025, 11:59 PM ET. We encourage vendors to submit their PY 2024 final data files as soon as possible.

Vendors are reminded to pay close attention to the feedback provided on their 2nd interim data submission and correct issues noted there, if any. In addition, vendors are encouraged to take advantage of the available resources which are key to successful data submissions: the XML file layout, the XML Schema File (XSD) and the Example XML File. Use the Schema Validation Tool once you have a complete XML file which you are ready to test by following the steps below:

Log in to your account on the ACO REACH CAHPS website: https://acoreachcahps.org
Select “Schema Validation Tool” from the Data Submission drop-down menu
Select the XML file you would like to validate
Click “Validate” to run the validation checks on your XML file. This application performs following checks:
- Well-formed test: Is your XML file a properly formatted file?
- Schema validation: Does your XML file match the ACO REACH CAHPS XSD Schema File?
- Data element range checks: Do the values for your data elements fall within the acceptable ranges?

Once you choose your file and click on the “Validate” button, the system will provide an onscreen report that tells you that the file passed all checks or indicate the specific data elements in the file which have errors. You may adjust and re-run your XML files through the validation tool as many times as you like. Be aware that no data from the Schema Validation Tool is saved on the ACO REACH CAHPS website.

Important Note: Since CMS will not accept any final files after January 13, 2025, at 11:59 PM ET, please make sure you check your validation information and data submission reports to verify that all your submitted files were accepted. If necessary, correct and re-submit all files before the data submission deadline.

The final data submission window for ACO REACH CAHPS Survey Performance Year (PY) 2024 will open on Monday, December 16, 2024. The deadline to submit final data is 11:59 PM ET Monday, January 13, 2025.

Log in to your account on the ACO REACH CAHPS website: https://acoreachcahps.org.
Select “Schema Validation Tool” from the Data Submission drop-down menu.
Select the XML file you would like to validate.
Click “Validate” to run the validation checks on your XML file. This application performs following checks:
Well-formed test: Is your XML file a properly formatted file?
Schema validation: Does your XML file match the ACO REACH CAHPS XSD Schema File?
Data element range checks: Do the values for your data elements fall within the acceptable ranges?

Important Note: CMS will not accept any final files after January 13, 2025. Therefore, take advantage of the time in December and January to create your XML files and use the Schema Validation Tool to test them. This will help ensure a successful data submission during the December 16, 2024 – January 13, 2025 window. Once you have submitted data, check your data submission reports to verify that all your submitted files were accepted. If necessary, correct and re-submit all files before the data submission deadline.

The ACO REACH CAHPS 2nd Interim Data Submission deadline is TODAY

The ACO REACH CAHPS 2nd interim data submission deadline for PY 2024 is TODAY, Monday, November 25, 2024.

We thank those ACO REACH CAHPS vendors who have already submitted PY 2024 2nd interim data for their client ACOs. If you have not yet submitted PY 2024 2nd interim data for your client ACOs, please submit your file(s) by 11:59 PM ET today, November 25, 2024.

Important Note: Since CMS will not accept any 2nd interim files after November 25, 2024, please make sure you check your data submission reports to verify that all your submitted files were accepted. If needed, correct, and re-submit all files before the data submission deadline.

For information about data file preparation and submission processes, please review Chapter 7 of the ACO REACH CAHPS Quality Assurance Guidelines (QAG).

The ACO REACH CAHPS Survey Coordination Team will review all submitted interim files immediately upon submission. We will run a series of edits on survey data to check for issues such as inconsistencies, patterns, or unusual values. If we identify any issues, we will contact the survey vendor and explain the problem(s), determine the correction(s) needed, and ask the vendor to correct the issues, if any, for the second data submission. Please see section 10.4 of the QAG for additional detail on the data review performed by the ACO REACH CAHPS Coordination Team.
