Reminder that GPDC CAHPS Quality Assurance Plan (QAP) is due by May 13, 2022

This is a reminder that training-approved survey vendors must submit their QAP for the Global and Professional Direct Contracting Consumer Assessment of Healthcare Providers and Systems (GPDC CAHPS) Survey no later than Friday, May 13, 2022.

Survey vendors must follow the format and required content provided in the online Model QAP Outline linked here: https://gpdccahps.org/Vendors/Model-Quality-Assurance-Plan-QAP.

To ensure prompt review and acceptance of your QAP, please submit a Word version (not a .pdf or other format) of your QAP online at the "Submit Quality Assurance Plan" link located under the "Vendors" tab once you are logged into the private side of the website.

Remember that you must also update your online GPDC CAHPS Vendor Application Form if you have changed key personnel and/or subcontractors, or other key information presented in your vendor application.
